Reconstructive technique

TCM: Total Corporal Mobilization in genital reconstruction

TCM — Total Corporal Mobilization — is an approach that mobilizes erectile structures to expand reconstructive possibilities in selected situations. The technique should not be presented as a universal indication for penile enlargement.

01What it is

What is the TCM Technique

A genital reconstruction technique described in a scientific publication.

TCM mobilizes the corpora cavernosa, structures of erectile tissue. This mobilization expands reconstructive possibilities. The technique was described by Ubirajara Barroso Jr. and colleagues in 2022, and its indication depends on the characteristics of each case.

02Indications and evaluation

Indications and evaluation

The indication is individual and does not automatically apply to every genital reconstruction.

The indication depends on the cause of the condition, available anatomy, previous surgeries, preserved functions and goals. In the context of gender affirmation, TCM may also be discussed as a mobilization approach applied to metoidioplasty.

TCM in metoidioplasty (Portuguese)

TCM may be discussed in the context of metoidioplasty and genital reconstruction for transgender men. It is not synonymous with metoidioplasty and its indication is not automatic.

03Risks and limitations

What needs to be preserved

Planning considers blood supply, sensation, sexual function, the urethra and tissue quality. Preserving function is a goal of the plan, not a guarantee.

Risks

Bleeding, hematoma, infection, pain, wound-healing problems, anesthetic risks, functional changes and the need for further interventions should be discussed. If urethral reconstruction is involved, its specific risks must be explained separately.

Limitations of the evidence

The available literature helps explain the technique, but does not support promises of a fixed gain, absence of complications or automatic transfer of results between different populations.

Total corpora mobilization for penile reconstruction

International Brazilian Journal of Urology · DOI 10.1590/S1677-5538.IBJU.2022.0177

The 2022 publication in the International Brazilian Journal of Urology described the use of TCM in three penile reconstruction cases, with follow-up of 6 to 24 months. As a small series without a comparison group, it does not establish definitive conclusions about safety or efficacy.

In 2026, a study published in a scientific supplement of the Journal of Urology evaluated TCM in relation to length, sexual function, genital self-image and quality of life, as part of the continuing investigation of the technique.

05Safety and decision-making

Safety, limitations and decision-making

All surgery involves risks and individual outcomes cannot be guaranteed. The indication depends on diagnosis, anatomy, health conditions, previous treatments, goals and the ability to complete the necessary follow-up. The consultation should compare expected benefits, risks, alternatives and the possibility of not operating at that time.
